Aidmed – System to measure patient vital signs

Order

Aidmed is a medical device (certified) designed to collect and analyze patient physiological parameters and consists of:

  • Aidmed One with additional equipment (Pulse oximeter BM2000D).
  • Mobile application Aidmed Health.
  • Aidmed Cloud telemedicine system – a server that stores and analyzes data from Aidmed One.

Use Case

The Aidmed One recorder is a portable medical device.

It is designed to be worn on the chest by the patient using an attached chest strap and an attached pulse oximeter. The recorder has sensors that collect the following biosignals:

  • Bioimpendance – measuring changes in chest volume;
  • Accelerometer – measurement of patient movement and position;
  • Pressure sensor – airflow through the nose/mouth;
  • Thermometer – skin surface temperature;
  • Microphone – volume level (sound pressure, such as coughing);
  • ECG – one discharge based on silicone electrodes without gel;
  • SpO2 sensor – pulse rate and oxygen saturation of the blood – wireless sensor

Mobile App (Aidmed Health) - allows the patient to view the measurement results

The system allows data to be transmitted from the registrator to the Aidmed Health application installed on the patient’s phone using BLE technology.

Data transmission is initiated by Aidmed One and occurs automatically when paired with Aidmed Health. The app provides a display of all data collected during the examination.

Aidmed Health sends data to the Aidmed.ai server at a fixed interval so that the result can be analyzed and interpreted, and a diagnosis of the patient’s condition can be made by a medical professional.

Platforma telemedyczna Aidmed umożliwia zarządzanie prowadzonymi badaniami związanymi z bezdechem sennym, holterem EKG i monitoringiem pacjentów z objawami COVID-19. Pozwala na dodawanie nowych pacjentów, zakładanie nowych badań i tworzenie ostrzeżeń dla pacjentów.

Ponadto daje możliwość przeglądania zarejestrowanych pomiarów, przeglądanie badań, uzupełnianie ich opisu i generowanie raportów. W oparciu o  sygnały system pozwala na automatyczną detekcję niepokojących zdarzeń  (parametry detektora mogą być modyfikowane przez użytkownika). Przykładowo Hypopnea (w oparciu o przepływ przez kaniulę oraz SpO2) czyli zmniejszenie przepływu o co najmniej 30% przez >10s, spadek saturacji o 4%.

Data Flow

The physician/healthcare professional agrees with the patient on the length and frequency of the measurements (e.g. 3x15min during the day, night measurement)

The examination will consist of recording the following signals of the patient:

  • ECG
  • SpO2 and HR  
  • temperature
  • cough (based on acoustic signal and information about chest movements)
  • respiratory movements
  • activity

AIDMED ONE recorder has several modules for measuring physiological parameters. The ECG signal is collected using a single-channel module through silicon electrodes placed on a chest strap.

This module is also used to detect changes in the electrical impedance of the chest. These occur during breathing – as the shape of the chest changes during inhalation and exhalation, and conductivity changes of the high-frequency current through the tissues. From these changes, the respiratory rhythm can be determined.

Another module is the differential pressure sensor. Using the intranasal cannula supplied with it, the air flow through the nose can be measured and the rhythm and depth of breaths can be determined.

The device also allows skin surface temperature to be measured using a contact temperature sensor. Another module is an inertial motion sensor, which allows to detect patient’s movements and chest movements, which can help to determine the breathing rhythm or cough.

The recorder also has a built-in microphone, useful for detecting coughing and snoring in the patient. A wireless pulse oximeter is included in the set, which allows to determine oxygen saturation in blood.

The collected data is sent to a cell phone and from there to a server, which can be accessed by a doctor/healthcare provider. Based on the collected data, the doctor can assess the patient’s status and progress in treatment.

Report

Measurement results and the detected event can be included in the patient’s documentation

For this purpose, a report can be generated after the doctor/health care professional has described the examination. The professional user can set parameters to control the detection of the listed events.

For example, for a sleep-disordered breathing (Apnea) study, the system allows automatic identification of the following events: Apnea, Hypopnea (shortness of breath), and desaturation.

Example of report

from the Sleep Disordered Breathing Examination contains the following information:

  • ODI-oxigene desaturation index number of desaturation events (by hour) determined in each examination in relation to the length of the signal to be analyzed for that hour;
  • Meab desaturation: Average of desaturation level for desaturation events by hour of examination;
  • Desaturation coefficient (DC): saturation variation function
  • Heart rate distribution: the distribution of heart rate values (HRV) for the examination;
  • T90:The proportion of time in the examination in which the saturation did not fall below 90%;
  • Saturation distribution: the distribution of SpO2 for the examination;
  • Activity share: the amount of time the patient spent in a given position.

Machine Learning

Automated Detectors (using AI and ML) help detect medical events

The use of artificial intelligence methods combined with remote observation can help in the initial assessment of a patient’s condition, screening for diseases, and identifying patients who could develop a severe form of disease. With our detectors, the physician/healthcare professional receives pre-described reports and alerts to make clinical decisions.

Possible applications

Apnea and heart rhythm

to assess the relationship of recorded heart rhythm disturbances to sleep-disordered breathing, which is essential for the correct choice of appropriate treatment of rhythm disturbances;

Apnea and comorbidities

For the follow-up of patients with suspected sleep-disordered breathing (SBS), especially obese patients, with suspected breathing disorders (e.g., chronic obstructive pulmonary disease – COPD), treatment-resistant hypertension, after CNS stroke, etc.

Primary Cardiac Diagnosis

Analyze the correlation of reported symptoms (such as palpitations, irregular heartbeat, shortness of breath, dizziness, fainting, loss of consciousness, fatigue) with the presence of arrhythmias and make a preliminary assessment of the type of arrhythmia;

Primary arrhythmia diagnosis

in the diagnosis of atrial fibrillation (AF); both in patients reporting symptoms suggestive of AF and in asymptomatic patients at high risk for this arrhythmia;

Differential diagnosis of atrial fibrillation

to evaluate arrhythmias and to differentiate between atrial fibrillation, arrhythmias of supraventricular and ventricular origin and, in case of bradyarrhythmias, to differentiate between sinus node automatic failure and atrioventricular conduction disturbances;

Apnea

The apnea and deflation detector detects declines in airflow through the cannula. In combination with other signals, our solution allows not only to diagnose but also to guide the patient more effectively in the therapeutic process.

Arrhythmia

Telemedicine sensors and detectors trained with machine learning techniques will be useful for diagnosis, but their predictive value for quantitative assessment of arrhythmia severity. It can be useful for therapeutic decision making and during treatment evaluation should be emphasized.
